For any victim of the Edenville Flood Damage that has suffered physical and financial damages as a result of this avoidable disaster:

Linked HERE please find a signature and verification form that you must complete and return to us as soon as possible so we can properly file your case in the Court of Claims. The deadline for filing your case in the Court of Claims is November 19, 2020. We must receive your signed and notarized form no later than Friday, November 13, 2020, to process and complete this filing with the court. The factual allegations related to your specific claim are also enclosed for your review and verification.

The only actions you must complete at this time are to sign the enclosed verification form, have your signature notarized, and return the form to us.

We are offering you several forms of assistance, described below, to ensure that you can complete this action properly before the deadline.

Please read and follow the below instructions carefully:

  1. Sign the verification form in ink.

  2. Have your signature notarized by doing any of the following:

    • Take the form to a bank or to another location offering the services of a notary public to have it notarized, and then hand deliver the signed and notarized form to us at our office located at 3139 N. Meridian Rd. Sanford, MI and is open Mon. - Fri. from 10:00 a.m. until 7:00 p.m.; OR

    • Bring the form to our office to be notarized. Our office is located at 3139 N. Meridian Rd. Sanford, MI and is open Mon. - Fri. from 10:00 a.m. until 7:00 p.m. If you need to make arrangements to come to the office at a different time, please send an email to contact@edenvilleflood.com or call our office at 989.289.8532 to make the necessary arrangements; OR

    • Contact us to schedule an appointment to have your signature form notarized via Zoom, FaceTime, or other remote technology. If you would like to arrange for a remote notary to assist you, please email your request to contact@edenvilleflood.com or call us at 989.289.8532.

  3. Do not send your signed form via U.S. Mail. Current delays in mail delivery services, especially during the election, could delay the delivery of your signed form. It is therefore essential that you follow the instructions set forth above to ensure that your verification form is properly signed, notarized, and received by us so that we can file your case before the court deadline.
